Early in the semester of anticipated graduation, students must complete a number of forms in order to prepare for completion of studies. These forms include the Masters Degree Audit form and an Application for Completion of Degree.

Masters Degree Audit Form

Early in the semester of anticipated graduation, each graduate student must complete a Masters Degree Audit form. This form requires you to provide a detailed listing of all courses you plan to use toward your graduate degree.

Masters Degree Audit Completion Instructions

Please read these instructions fully and follow them closely while completing your Masters Degree Audit form.

There are two versions if the Masters Degree Audit form on the Graduate School website. You should use the .pdf version if you have a full version of Adobe Acrobat or the Word version if you do not.
Type in your name and UID on page 1 of the form.
Skip to page 3.
Complete the personal information at the top of page 3.
Your major is Information Systems, and you should know your sequence name (Enterprise Computing Systems, Information Assurance and Security, Internet Application Development, Systems Development, OR Telecommunications Management).
Check the M.S. box and also check whether you are completing the course option ("30-39 hour option"), the thesis option ("with thesis") OR the project option ("with project or culminating experience").
Complete the Transfer Course section only if you are transferring graduate-level courses into the program from another U.S. University.
In the ISU Graduate Courses column, include all courses that will count toward your master's degree, including those in currently in progress.

Please note that you can look up all of the courses you’ve taken on iCampus to help you fill out the form.
BE SURE TO USE THE DEPARTMENT NUMBER (not the acronym, "IT") when listing your courses. Our Department Number is 515. The Technology Department’s Number is 510, which is only needed if you took TEC 349 to fulfill your degree requirements.

Enter the total number of graduate hours that will count toward your degree, total hours transferred, and the total number of 400-level classes you completed (not including IT 499 or IT 494 hours).
When you are finished completing the form, save it with your name in the file name.
Send the form to the IT Graduate Advisor as an attachment.

Application for Completion of Degree

Early in the semester of anticipated graduation, you must complete an Application for Completion of Degree and pay a graduation fee. The form should be completed and either dropped off with payment at the:

The exact deadline for submission may be found in the Important Dates and Deadlines document on the Graduate School website.

Reapplication for Completion of Degree

If you apply for graduation but do not graduate as planned, your Application for Completion of Degree will be cancelled.

You must reapply for a future semester by calling the Graduate School at 309-438-2583 or going in person to Hovey Hall 309 early in the semester during which you now plan to graduate. The Graduate School will update your application without any additional fee.

A new Masters Degree Audit need not be completed unless significant changes to your course of study were made after the Masters Degree Audit was submitted and approved by the Graduate School.

Commencement

Participation in Commencement is optional, but you must indicate on your Application for Completion of Degree whether or not you wish to participate. Additional information regarding commencement may be found on the Graduation Services website.

Students completing degrees in May or August are eligible to participate in the May Commencement Ceremony. Students completing degrees in December are eligible to participate in the December Commencement Ceremony.
